"Create" Coordinator (Access Creation Coordinator)
Location: Atlanta, GA Department: PVG – Atlanta ASC's Reports To: Clinic Manager FLSA Status: Exempt
Essential Duties and Responsibilities
- Receive referrals for initial access creation for all GA ASCs
- Develop and maintain a detailed inventory of patients in various phases
- Communicate with physicians, ASCs, and patients to ensure efficiency
- Personally guide each patient through the access creation process
- Build relationships with stakeholders to facilitate care coordination
- Responsible for careful tracking of all relevant data to closely monitor program performance
- Prepare reports on program performance and effectiveness
- Develop working relationships with dialysis units and nephrologists
- Responsible for requesting and obtaining appropriate information from referring facilities
- Manage hospital surgery schedules, labs, and cardiac clearances
- Clear all patients on surgery schedule with preauthorization verification
- Ensuring all hospital patients' medical records have been input into system
- Work with MD to ensure timely hospital billing completion
- Obtain insurance cards and picture ID; confirm insurance information
- Responsible for assuring referral paperwork is complete on all patients
- Obtain valid prior authorization from insurance companies
- Communicate scheduled patient information to intake coordinators
- Manage no-shows and reschedules appropriately
- Track patients pre and post-surgery; capture critical clinical data
- Fax reports to dialysis centers and physician offices as needed
- Assist clinical staff in obtaining data for required reports
- Maintain strict confidentiality of PHI and protected information
- Comply with adverse incident reporting system
Qualifications
- Minimum of 2 years in a medical or office setting required
- Basic medical terminology and knowledge of medical specialties
- Preferred: 2+ years experience in dialysis, interventional radiology, cardiac cath labs, or diagnostic imaging
- Strong computer skills are essential including Microsoft Word and Excel
- Strong organizational skills with the ability to multitask
- Strong interpersonal skills
- High school diploma or GED
